Abstract Introduction Brucella epididymo-orchitis is a complication that occurs in 5.7% of patients with brucellosis and can rarely mimic testicular tumor. We here report case 25-year-old man presented as mass responded to conservative treatment using antibiotic therapy. Case A patient left pain, abdominal easy fatiguability, feeling unwell. Physical examinations were normal. tumor was suggested by scrotal color Doppler US scan MRI. However, markers all within the normal range. serum agglutination test (Rose Bengal test) done, it positive. The received anti-brucella antibiotics. An ultrasound 3 months later revealed complete recovery after completing treatments. Discussion orchitis be determined based on triad serology, ultrasonography, presence common symptoms like fever, redness, enlargement. early diagnosis this phenomenon crucial due morbidity complications may encountered. Moreover, has several differential diagnoses, such tumor, epididymitis, trauma, torsion testis, hematocele. Failure obtain an adequate diagnosis, particularly if suspected, result unnecessary intervention orchiectomy. Conclusion brucella infection Several techniques are required for accurate assessment.
